We are the only agency providing this service in Barrington, Hanover, Palatine and Wheeling Townships.
How it works
A free consultation and assessment is provided by a professional Case Manager to all individuals prior to admission into a nursing home.
This assessment is known as the Universal Pre-Admission Screening and is required for all stays at a nursing home whether for short-term rehabilitation or long-term stays.
The Case Manager will come to your home, or if you are in the hospital, he or she will visit you there.
The purpose of this assessment is to assure that placement into a nursing home is the most appropriate and affordable choice for the person who needs care.
Case Managers will ask questions about your ability to perform certain tasks such as:
- Cleaning
- Shopping
- Preparing Meals
- Managing Medications
- Managing Finances
- Getting around the house
The Case Manager will also ask some questions to help assess any memory difficulties.
