
The Community Care Program (CCP) is designed to:
- Help senior citizens remain in their own homes
- Prevent premature placement of seniors into nursing homes
- Help seniors maintain their independence by providing in-home and community based services
We are the only agency providing this service in Barrington, Hanover, Palatine and Wheeling Townships.
Services Provided Through CCP Are:
- Adult Day Services
- Homemaker
- Money Management
- Emergency Home Response Systems
How it works
Contact our Intake department at 847-253-5500. Our intake department will talk with you and find out what your needs are.
If appropriate, you will be sent an application form for CCP that will be returned to our office.
A Case Manager will contact you and arrange a home visit.
During the home visit, the Case Manager will discuss your needs with you and determine whether you qualify for the Community Care Program.
Eligibility Requirements
Your Case Manager will help you determine if you are eligible for CCP services.
You are eligible if you:
- Are age 60 or older
- Are determined physically or cognitively in need of service
- Are residing in the state of Illinois
- Are a United States citizen or legal alien
- Meet the Asset Requirements for CCP
- Apply for medical benefits, if required based on your financial situation
If you qualify for CCP, a professional Case Manager will work with you to:
- Determine your needs
- Set up appropriate services
- Keep in touch with you to make sure the services continue to meet your needs
Asset Requirements
- You cannot have assets having a combined value in excess of:
- $17,500 for an individual
- $17,500 for each spouse if both are receiving services
- $101,640 for a married couple when only one receives services. (Requires a Public Aid application for Prevention of Spousal Impoverishment or PSI.)
However, not all of your assets are counted.
Assets which are not counted:
- Home in which you live
- Most personal belongings
- Car
A Case Manager will explain any other exceptions.
How Much Does it Cost?
Your share of the cost is on a sliding scale based on your income:
If your family income is below the federal poverty level, the state will pay the entire cost of the service. You will not be charged a fee.
If your family income is above the federal poverty level, the state may still pay part of the cost of service.

Adult Day Services
Adult Day Service offers:
- care and supervision of clients
- in a group setting outside of their own home
What Do Adult Day Services Provide?
- Exercise therapy
- Opportunity to meet others
- A nutritious noon meal together
- Social activities such as:
- Arts and Crafts
- Card Games
- Current Event Classes
- Bathing assistance depending on the site
- Respite for family members who work during the daytime
Transportation to the day center can be provided or arranged. You can discuss the specifics with your Case Manager or Adult Day Care staff.
Some Adult Day Service centers funded by IDOA offer specialized services for individuals who suffer from Alzheimer's disease or related disorders. Ask our Intake department for more details.

Homemaker
Homemaker Services offer non- medical support in the seniors home.
Each homemaker is supervised and has received specialized training.
Homemakers are available to:
- Dust
- Vacuum
- Clean the kitchen and bathroom
- Do laundry
- Prepare meals
- Remind client to take medication
Homemakers can also assist with or perform:
- Grooming
- Bathing
- Dressing
- Meal planning
- Household supervision
Services are based on a free Plan of Care developed by a professional Case Manager.

Emergency Home Response Service
The Emergency Home Response System:
- Can signal for help anytime
- Two-way voice communication system
- Improves safety

Money Management
The Money Management Program Can Provide:
- Assistance to low-income seniors
- A rest/break time for the family
- Companionship
- Help opening and sorting mail and bills
- Help keeping a balanced checkbook
- Independence
Reasons Persons May Need Money Management
- Physical or Mental Impairment
- Bills are not being paid
- Client has difficulty reading or writing
- Client is overwhelmed or nervous
- Difficulty managing personal household budgets
- Difficulty keeping track of banking records
- Loss of support or caregiver
- Financial exploitation or abuse
- Serious illness
- Client self-neglect
- Utilities shut off
How it Works
Volunteers are selected and trained to assist seniors in managing their finances. Volunteers must provide references and a criminal background check is completed.
AARP:
- Provides insurance coverage of client funds
- Provides program policies, procedures and materials
Clients are referred to us by:
- Social Service Agencies
- Social Security Administration
- Family/Friends or other concerned persons
Once a referral is made, a trained volunteer and local program coordinator meet with the client to:
- Explain the program
- Assess client's needs
- Determine if the client is eligible
What the Volunteer Money Manager Will Do
There are 2 basic types of volunteer programs:
- Bill Payer
- Representative Payee
Bill Payer
A Bill Payer Volunteer will provide:
- Opening /organizing mail
- Checkbook balancing
- Bill paying services for seniors who remain in control of their finances but need some help keeping things in order
Representative Payee
A Representative Payee Volunteer:
- Is appointed by the Social Security Administration
- Manages monthly benefits of a client determined incapable of handling his/her own finances
- Manages and pays the client's expenses from a separate account that is opened where the client's benefit checks are automatically deposited
Program Safeguards
- Insurance coverage of client funds is provided by AARP
- Volunteers are carefully selected, trained and receive ongoing support
- Volunteers only work from one designated account with a $3,500 limit
- Volunteer activity and accounts are monitored on a monthly basis
