
A Professional Case Manager is assigned to help:
- Assess Needs
- Develop a Plan of Care
- Coordinate Services
How It Works
When a senior is in need of a particular service that we offer, he/she will be referred to a professional Case Manager.
The assigned Case Manager will contact the client and arrange a time to meet.
The Case Manager will then provide a free comprehensive assessment in the senior's home.
The Case Manager will keep in touch with the client to make sure his/her needs are being met.
For as long as the client is receiving services, the Case Manager will re-asses the client's needs on a yearly basis, or more often as needed.
During the home visit the Case Manager will:
- Assess the client's physical, emotional, cognitive, and financial needs
- Review service options and costs, if any
- Develop a Plan of Care
- A Plan of Care is a summary of what services may be needed and how they will be met.
Our Case Managers have:
- Experience arranging services to help seniors remain in their homes
- Resource lists based on the individual needs of the senior
- General information on nursing home care if needed
