Provide direct service to family caregivers of elderly clients. Completes home visits in the community. Co-facilitates family caregiver support groups. Teaches cleasses to family caregivers. Gives presentations to community groups on family caregiver issues. Represents agency at community outreach events. Identifies family caregivers and connects them with services. Coordinates services for family caregivers from care providers. Experience with older adults and their family caregivers preferred. Caseload of complex clients with multiple needs. Created effective Care Plans, authorizes governmental services, refers and links family caregiver clients to community resources, and monitors client services with supervisory guidance. Proficient with Microsoft Office: Outlook/Excel/Word. Accurate and timely recording and tracking fo activities performed in multiple computer systems for funding bodies. Ability to meet deadlines. Occasionally up to four hours weekly required outside business hours. Follows the standards and guidelines of the Illinois Department of Aging, the Area Agency of Aging, Housing and Urban Development (HUD) and the Council on Accreditation. Serves Barrington, Hanover, Palatine, and Wheeling Townships.
