AssessorResponsible for completing alcohol/drug assessments inclusive of DUI, Non-DUI, Secretary of State Updates, Letters of Denial, Investigative Reports and misdemeanor cases in accordance with DASA, Illinois Secretary of State, CSIA and Circuit Court of Cook county rules and regulations. These assessments may be conducted within the regional office or in Department of Correction’s facilities. Completes intake processing, inclusive of data entry. Knowledge of and ability to conduct specimen collections for toxicology screening and represents alcohol/drug assessments at the Secretary of State hearing facilities as designated by the manager. May be required to provide evening, weekend and out of venue services as deemed appropriate by CSIA Management. Will be required to work with at risk population.

Case Manager, Service CoordinatorProvides direct service to elderly clients needing assessment and Case Management to remian safely and comfortably in the community. Completes home visits in the community, hospital, or in HUD 202 buildings. Conducts assessments; provides information; develops realistic Care Plans; authorizes governmental services; refers and links clients; and monitors client services. Follows the standards and guidelines of the Illinois Department of Aging, the Area Agency on Aging, Housing and Urban Development(HUD) and the Council on Accreditation. Utilizes computer systems; types service logs and case notes; and enters data electronically.

Doula Case ManagerProvides Doula services to pregnant teens assuring that the clients receive emotional and physical support during pre-natal, intrapartum and post partum activities. Is present during the child birthing expereince. Promotes health and fitness through delivery of educational groups. Is available and on-call to clients 24 hours a day.
