This outreach program is funded by the Chicago Department of Public Health to reduce the occurrence of vaccine-preventable diseases through a comprehensive screening and assessment of the immunization coverage levels of low-income infants and children up to age two. Operates in nine Public Health/WIC clinics in Chicago.
