Bridge Subsidy Program


In partnership with the Illinois Department of Human Services, Division of Mental Health, Catholic Charities works as a subsidy administrator for the Division of Mental Health's statewide Bridge Subsidy Program. This program is designed to provide eligible Division of Mental Health consumers with help paying rent on an apartment in the community, while also providing them access to a wide variety of supportive services. Catholic Charities operates as subsidy administrator in Cook and Lake Counties.

Family Shelter Program


Provides interim housing to families at two locations in Chicago. Catholic Charities' shelters services both one- and two-parent families, with children up to age 18. Heads of household must be at least 18 years of age. Residents are given basic needs, social services, counseling, and referral to other services as needed. Maximum length of stay is 120 days.

The HPRP (Homeless Prevention Rapid Re-housing Program) is focused on housing for homeless and at-risk households. The program provides temporary financial assistance, housing relocation and stabilization services to individuals and families who are homeless or would be homeless without this assistance. This is a temporary program funded through a grant from the U.S. Department of Housing and Urban Development and the American Recovery and Reinvestment Act (ARRA).

The Homelessness Prevention Call Center is a key component in community efforts to end homelessness. It is housed and operated at Catholic Charities. Services are available in multiple languages. The Call Center's Information and Referral Specialists screen callers experiencing a short-term, one-time crisis and determine if fund resources are available such as state, municipal and county programs, as well as private funds. Eligible callers are referred to the appropriate social service agency that will process their application. Resource providers and agencies benefit from the call center’s process of intake and criteria-matching, while the community benefits from a centralized source of advocacy data.

In the City of Chicago, residents can call the Chicago City Services 311 number and ask for "Short-Term Help." Chicago's 311 City Services operator will transfer the call directly to the Homelessness Prevention Call Center. In the City, the program is a collaboration between the Chicago Alliance to End Homelessness, Catholic Charities, the City of Chicago, Emergency Fund, privatefoundations, homelessness prevention advocates and local social service agencies.

Non-Chicago residents who reside in the Cook County suburbs can access the Call Center's services by calling toll-free (877) 4-COOK-15 (877-426-6515). In the suburbs, the Homelessness Prevention Call Center is a collaboration between Cook County, the Suburban Alliance to End Homelessness, the Village of Oak Park, the City of Berwyn, the City of Cicero, Emergency Fund, private foundations, homelessness prevention advocates, and local social service agencies.

Comprehensive housing counseling and case management assisting the homeless and those at riskof homelessness in locating and securing permanent housing. Services include housing/tenant education, advocacy, information and referrals. Provides mortgage default and foreclosure prevention counseling. Provides financial literacy education and budget counseling for clients experiencing financial difficulties. Services by appointment in four locations in Lake County. Services are available in Spanish.

New Hope Apartments


Catholic Charities New Hope Apartments provides housing with intensive case management services for homeless families with children under the age of 18 in both the City of Chicago and suburban Cook County. Families are housed in scattered site transitional apartments and, for up to 24 months, receive services that include rental assistance, household furnishings, employment assistance with training and educational referrals, child care, budgeting assistance, transportation and assistance in accessing appropriate government programs.

Through Individual Action Plans, clients focus on barriers to job placement. Families achieve job skills while working to increase income, decrease debt and increase self-determination during their time in the program. Additionally, in North/Northwest Suburban Cook County, families can receive up to nine months of emergency housing and case management services in scattered site apartments.

In suburban Cook County, New Hope Apartments provides Permanent Supportive Housing for homeless families in which the head of household or spouse has a disabling condition. In North/Northwest Suburban Cook County, New Hope Apartments works in partnership with the Alexian Brothers Center for Mental Health in providing Permanent Supportive Housing for homeless individuals who have a disabling condition. Permanent Supportive Housing consists of rental assistance along with supportive services, and continues so long as participants continue to meet eligibility requirements.

SAFE Housing Program

The SAFE Housing Program is a two year transitional housing program for recovering, homeless women and children with, or at risk of, Department of Children and Family Services involvement. Supportive services enable families to increase their capacity to effectively cope with multiple life stressors, co-occurring behavioral/mental health and substance abuse issues. Intensive case management, counseling and employment services focus on developing self-sufficiency, parenting skills and maintaining sobriety to gain permanent housing.

St. Leo Residence


St. Leo Residence is a long term transitional residence including 141 individual studio apartments for single individuals giving preference to homeless veterans. The building includes a computer library, community rooms, laundry facilities, and 24-hour desk clerk, as well as on-site supportive services with three case managers and a community liaison assisting residents with adjustment to housing, local community integration and social service needs. Residents will also have access to a Community Health Care Clinic for outpatient community-based health care, job services through the Department of Labor and a private garden donated by the Illinois AMVETS and dedicated to living veterans.

Streets To Home Initiative


The Streets to Home Initiative is funded through a grant from HUD and DFSS. This provides a full-time case manager who places up to 25 chronically homeless individuals in housing and provides intensive case management services with a focus on assisting the individuals in maintaining their housing.
