Care Coordination

Our care coordinators provide person-centered, in-home assessments to determine the level of care seniors need to remain safely in their homes and prevent premature nursing home placement. Our goal is to help seniors age in place, living independently or with caregivers for as long as possible.

We work with individuals, families, and community partners to develop a plan of care to assist seniors with daily activities in the home and community.

  • Access to Home-Delivered Meals, Chore Housekeeping, Caregiver Services, and Benefits Assistance
  • Care Plan Development and Monitoring
  • Nursing Home Pre-screening
