Service Coordination

Catholic Charites’ service coordinators provide onsite assessments, information, and referrals for seniors living in affordable housing residences across Cook County.

Our service coordinators connect older adults to critical services so that they can continue to age in place while staying connected to community. They also educate residents about their rights and responsibilities.

We provide this service for income-qualifying seniors living at 52 affordable housing buildings managed by the Chicago Housing Authority throughout the city. We also serve residents at our 19 buildings managed by CCAC in Chicago and surrounding Cook County.
